COVID-19 INFORMATION FOR CLIENTS

On March 24th the Douglas County Health Department required local tattoo studios to shut down operations in an effort to “flatten the curve” and reduce the upsurge of Covid-19 in our community. On May 4th, Governor Ricketts deemed that our industry could resume business as long as we follow Nebraska’s DHMs and necessitate masks for artist & client. With much consideration, our staff felt we needed to further research best practices in order to keep ourselves and our clients safe. We chose to extend our opening date to May 18th, 2020. We are now operating at a lower than normal capacity, but we are open!

During our downtime, we spent many hours in the studio expanding the artist space for distancing, sanitizing the studio by professional cleaners, and plotting a new system of operating to make our studio one of the safest tattoo shops you could visit. New arrangements include: going paperless, staggering schedules, following a disinfecting schedule, etc. Knowing that the threat of Coronavirus may be an extension of our daily routine, we’re sharing some of the new shop guidelines for clients during this time.

COVID-19 Pandemic Client Consent Information:

I confirm that I am not presenting any of the following symptoms of COVID-19 listed below:
– Cough
– Shortness of breath

-Fever – Muscle Pain – Headache – Dry Cough – Repeated shaking w/chills – Loss of sense of taste or smell

I verify that I have not traveled domestically within the United States by commercial airline, bus, or train within the past 14 days. If so, I will notify the staff at time of booking or 3 days prior to arrival.

I understand and acknowledge that people of any age who have serious underlying medical conditions might be at higher risk for severe illness from COVID-19.

To prevent the spread of contagious viruses and to help protect each other, I understand that I will have to follow the studio’s strict guidelines (ie: i will need to wear a face covering or mask the entire time I am in the facility..etc)

I understand that the CDC, OSHA, & the Nebraska Department of Health and Human Services recommend social distancing of at least 6 feet.

I will not bring guests or unnecessary items into the studio to reduce the risk of virus spread.

I understand that due to the frequency of visits of other clients, the characteristics of the virus, and the characteristics of tattoo services, that I have an elevated risk of contracting the virus simply by being in the studio.

I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not given the current limits in virus testing.

I understand that Black Squirrel Tattoo LLC has the right to refuse service to anyone who violates policies related to the health and safety of their customers and employees.
